Dr. Lorenzana is the President and Director of Lorenzana P.C. dba. ADVANCED VISION CENTER, |

located in Streamwood, Illinois. Advanced Vision Center is first and foremost a Primary Eyecare Center. |

However, because of Dr. Lorenzana's unique educational and professional achievements, patient |

services at the practice have been expanded to include complex subspecialty care. |

In 2002 Doctor Lorenzana was granted membership into the Pediatric Eye Disease Investigative |

Group (PEDIG), where she remains an active principal investigator. In 2004 (JAEB HRC) certified |
| |
Advanced Vision Center as a clinical research site for the Amblyopia Treatment Study. At that time, |
| |
Dr. Lorenzana received an "Outstanding New Clinical Site Award". Dr. Lorenzana is a well-known |
| |
lecturer has been published in many scientific journals that include the Journal of Neuro-cognitive |
| |
Rehabilitation, Behavioral Optometry and Optometric Management. |

About Dr. Lorenzana |
| |
Born in Bogotá Colombia she moved to the United States when she was ten. She feels privileged |
| |
to have completed her professional education in the USA, which she still refers to as "The land |
| |
of opportunity". She actively participates as an optometric consultant to organizations such |
| |
as Casa Central Head Start, Children's Home and Aid Society and La Fayette Specialty Academy, |
| |
as well as serving as a member of the Board of Directors for Rainbow House, a non-profit institution |
| |
that serves battered women and their children. Dr Lorenzana's personal struggle to learn English |
| |
as a second language when she was very young enables her to identify with people of all kinds |
| |
who encounter challenges on day-to-day basis, no matter there origin. She has a passion for |
| |
serving the eye care needs of children and patients in the Hispanic community. |

For over 35 years, Dr. David Sacks practiced optometry in what was once referred to as the Tri-Village |
| |
area. He moved his practice to its present location 12 years ago, where he continued to provide |
| |
excellence in eye and vision care services to the residents of Streamwood, Hanover Park, Bartlett and |
| |
the surrounding Northwest Suburban area. |
| |

"A family practitioner," Dr. Sacks performed a wide range of ocular care services to people of all ages. |
| |
Early on in his career, he was an active member of the Streamwood Chamber of Commerce and received |
| |
recognition from Elgin Community High School for his continued support of students seeking |
| |
vocational training. |

Over his entire career, Dr. Sacks examined over 50,000 patients, and feels a debt of gratitude to the |
| |
residence of the Tri-Village area for their loyalty and support. |
